Why a Bad Night Amplifies Your Pain: What Neurophysiology Says

In the first article of this series, we established that sleep and pain influence each other in both directions. But 'influencing each other' stays abstract until you understand what actually happens in the body during a night of poor sleep.

That's what I want to unpack here — not to turn everyone into a neurophysiology expert, but because understanding the mechanism often changes how people approach the problem. Many of my patients tell me that once they understand the 'why,' they stop feeling guilty or searching for a purely mechanical explanation for their pain.

 

A Nervous System on High Alert

Deep sleep is one of the moments when the autonomic nervous system shifts into its 'rest and digest' mode — the opposite of the 'alert' mode triggered by stress. When sleep is fragmented or insufficient, that shift doesn't happen properly, and part of the nervous system stays on higher alert than necessary, even during the day.

A nervous system that stays on alert tends to treat more of the body's signals as potentially threatening — including sensations that, in a rested state, would be perceived as neutral or unimportant.

 

Central Sensitization: When the Brain Amplifies the Signal

This phenomenon has a name in pain research: central sensitization. It's not a worsening injury — it's a central nervous system that becomes more reactive, amplifying signals that used to go unnoticed.

Lack of sleep is one of the factors that can encourage this phenomenon in some people. It's neither automatic nor universal — but it's a serious lead for explaining why the same 'physical' pain can feel much more intense after a restless night.

 

The Role of Cortisol and Low-Grade Inflammation

Sleep also plays a role in regulating cortisol, one of the stress hormones. Chronically disrupted sleep is associated, in many people, with a dysregulation of this hormonal rhythm.

It also plays a role in regulating certain low-grade inflammatory markers — a subtle inflammation that isn't visible or directly felt, but that can contribute to maintaining pain sensitivity over time.

None of this means the pain is 'in your head' — quite the opposite. These are real, measurable mechanisms that explain why sleep is never a minor detail in managing chronic pain.
